1. How will metoprolol help to reduce the patient’s blood pressure? (Hint: mechanism of action).
Answer – Metoprolol is a competitive inhibitor of cardioselective beta-1-adrenergic
receptor. It block  β 1-receptors in the heart muscle with negligible effect on β 2 receptors. Metoprolol reduces the heart output by negative inotropic and chronotropic effects with no action towards membrane stabilization nor intrinsic sympathomimetics..  Metoprolol is known to reduce heart rate. The drug is administered either orally by intravenously. When consumed orally it is completely is absorbed in the gastrointestinal tract. The drug is largely lipophilc . It is effective when consumed once per day. ( Morris & Dunham, 2018). Metoprolol is also used It is used to treat  chronic Angina due to insufficient blood circulation to the heart, and various other medical conditions involving such as Supraventricular Arrhythmias, Non-ST-Segment-Elevation Acute Coronary Syndromes (NSTE ACS), Acute MI, Ventricular Arrhythmias etc.
2. What adverse reactions should the patient be aware of? Link the adverse reactions to the mechanism of action.
Answer-  The main side effects of metoprolol involved is heart failure exacerbation, depression, fatigue, heart block, hypotension, bronchospasm, cold extremities, dizziness, decreased libido, diarrhea, tinnitus, decreased exercise tolerance, glucose intolerance and may also mask hypoglycemia. This is due to the reason that the blockage of cardioselective beta-1-adrenergic receptor with beta-blocker medications causes many unfavorable effects. Sudden termination of the drug  causes withdrawal syndrome such as angina or myocardial infarction. Tachycardia and hypertension is also common withdrawal syndrome after stopping the drug.
