Case Management in the Community
Some case management programs are community based and begin after clients are discharged from the hospital. These services are provided by nurses and other members of the interdisciplinary team. Clients are assessed, counseled, reminded of appointments for follow-up, and referred to other professionals as needed. These services are frequently provided for behavioral health clients who are reintegrating into the community. These services may be provided by the health maintenance organization (HMO) case manager, who is accountable for coordination of care overall, or by other case managers who manage “crisis and details while that patient resides in their respective level of care” (Powell, 2000a). These case managers specialize in areas such as home health, skilled nursing, and rehabilitation.
