Endoscopy Report


Pt: Female, age 89 Dx: Gastrointestinal hemorrhage, duodenal ulcer Tx: EGD Ms. Dena Jitters was admitted through the emergency room because of vomiting coffee-grounds material, and passing melenic stools. The nasogastric tube was introduced and showed bright red blood. The patient became hypotensive and two units of packed red cells were given. Ms. Jitters became stable with a BP of 120/80. She was lavaged with iced isotonic saline and an endoscopy was performed. REPORT: Premedication: Cetacaine locally. The endoscope was easily passed into the esophagus. Numerous amounts of thrombi were noted. The scope was introduced into the stomach, which showed increased amounts of bright red blood in the fundus. No mucosal lesions could be seen; however, half of the stomach was full of blood. The scope was then passed into the duodenal bulb, where clots were also noted. This patient could have a duodenal ulcer but because of the amount of blood, it is difficult to delineate an ulcer crater. A great clot was located on the anterior wall. As the scope was withdrawn, the antrum of the stomach could be seen with no lesion noted. It was difficult to clean up all the blood clots and, because of the status of the patient, the procedure was discontinued. Ms. Jitters tolerated the procedure well and did not vomit or aspirate.
