In this unit students will gain an understanding of the mental health system in Australia and the reforms that have contributed to the current mental health policy and service landscape. Students will become familiar with classification systems such as the Diagnostic and Statistical Manual 5 (DSM-5) and learn how to access information as required on particular mental health disorders. This unit will provide learning on the psychologically focused strategies that have been identified as effective for a range of mental health disorders. Through theoretical and practical learning students will critically evaluate national mental health policies, service delivery, prevention and early intervention programs. Self-awareness is a key aspect of learning in the field of mental health. Stigma and discrimination have been associated with mental illness therefore students will be invited to reflect on their own attitudes and beliefs related to mental health. Students will engage with mental health services to gain insight into consumer perspectives on experiences of mental illness and on treatment options. The emphasis of this unit is to equip students with the knowledge and skills to identify a client with a mental health problem, engage in minor assessment tasks and make appropriate referrals. Introduction
Stigma and discrimination against people suffering from mental illness has been a common occurrence since ancient tim
es. Earlier on, people with mental illnesses were considered mad, possessed by spirits or demons and witches. They were shunned from society, ignored and neglected. The insecurity, discomfort and threat people felt towards those with mental illnesses made them exclude and discriminate such individuals from social circles and society.
Stigma and Discrimination have been distinguished from each other on multiple bases. However the root difference between the two is that stigma is a perception, a way of looking at people with mental illness, while discrimination refers to treating people with mental illness in a different manner than one would treat a normal person. This includes ignoring, providing lesser opportunities, lower pay rates etc.
Stigma and discrimination hurts people suffering from mental health problems. It reduces their self-esteem, makes them more reluctant to look and respond to treatment. Stigma and discrimination creates feelings of shame and exclusion. It may also lead to depression and introvertness, thereby further reducing chances of treatment and cure for patients.
Stigma and discrimination against individuals with mental illness is fairly common all over the world. Australia is no different with an estimated 2.5% facing some or the other sort of stigma and discrimination in the country (ABS, 2008). This paper discusses stigma and discrimination against the mentally ill and its effect on the Australian population and the various initiatives the government is taking in controlling and addressing this grave issue. It also offers recommendations to improve government initiatives and effectively reduce stigma and discrimination.
Prevalence of stigma and discrimination
The rate of mental illness is high in the Australian population. Reports show that every one in five person between the ages of 16-85 has some form of mental illness including mood disorder, anxiety, depression or substance abuse. 14% of children between the ages of 4-17 were found to have a mental illness (The Department of Health, 2009; Australian Bureau of Statistics, 2010).
The Report of the National Inquiry into the Human Rights of People with Mental Illness states that
“People with mental illness experience stigma and discrimination in almost every aspect of their lives” (Human Rights and Equal Opportunity Commission, 1993, p.925).
In order to understand the extent of impact of stigma and discrimination, it is necessary to understand what these stigma and discrimination really mean.
Stigma can be defined as disapproval of a person due to that person’s difference from others in any form leading to discrimination against them. Stigma can broadly be classified into two forms, public stigma and personal stigma (Stuber, Meyer & Link, 2008).
Public stigma refers to the prejudice of the general public and the resulting discrimination it creates by the people towards those suffering from mental illnesses.
Personal stigma is one that relates to the person with the mental health condition. It comprises of perceived stigma, experienced stigma, and self-stigma.
Perceived stigma is an individual’s anticipation of stigma that he is going to face from the people as a result of his/her illness. Those suffering from mental illness realize that they are living among people who will stigmatize them. This knowledge lowers their already low self-esteem and devastates their confidence. Cultural stereotypes that exist in today’s societies and which these individuals are aware of worsens their perception of their self and induces hopelessness and frustration in their lives.
Experienced stigma is the actual discrimination that is experienced by people with mental illness. This not only injures the person with mental illness, it makes them more susceptible to depression and pain.
Self-stigma or internalized stigma of mental illness happens when an individual with mental illness accepts and adopts the stereotype held against him. This individual transforms his identity and becomes negative regarding his own life. He loses positive beliefs about the self and becomes highly introverted. He stops seeking help from others and starts to internalize the stigma as part of his own personality. Such individuals seclude themselves, and stop participating in societies to avoid the stigmatised reaction of people around them (Bathje & Pryor, 2011).
Discrimination on the other hand refers to the unfair or differential treatment of certain individuals based on their peculiar age, race, sex or health condition (Stuber, Meyer & Link, 2008).
