The impaired gas exchange (hypoxemia) and ineffective breathing pattern develops in COPD patients due to ventilation perfusion mismatch, progressive limitation of airflow and dysregulated ventilation (Sarkar, Niranjan & Banyal, 2017).
Mr. Krum is a current cigarette smoker and has a past history of emphysema. It means his alveoli are damaged and the alveolar spaces are permanently enlarged. These changes are irreversible and occur due to proteolytic damage of alveolar matrix (Sarkar et al., 2017). The alveolar damage lowers down the elastic recoil property of the lungs, which further reduces their ability to open up the minute airways (Madan, Gupta, Mittal & Chhabra, 2016). Emphysematous airways have lost their alveolar walls, their alveolar spaces get enlarged and their alveolar wall attachments get decreased (Cantor & Turino 2019). It damages the pulmonary microcirculation, involved in the exchange of gases (Madan et al., 2016). In this case, the long term exposure of lungs to cigarette smoke is the main cause of lungs and airways damage. The injured alveoli have problem in moving adequate air through the lungs, which makes it difficult to breathe (Madan et al., 2016). The patients of COPD having emphysema, are at greater risk of weight loss and dyspnea.
Mr. Krum has a history of malnutrition. In respiratory distress, the patients are often anorexic due to sputum production, dyspnea and medications (Cantor & Turino, 2019). The respiratory inadequacy increases the calorific demands in the body. Therefore, COPD patients often have malnutrition symptoms. The smoking may also cause inflammation of small airways which are recurrently exposed to smoke (Mete, Pehlivan, Gulbas & Gunen,2018). The dysfunction of small airways gradually gets severe in form of emphysema. The imbalanced intake of nutrition less than the normal body requirements may be related to dyspnea, anorexia, production of sputum and reduced food intake due to fatigue (Goldklang & Stockley, 2016). The patient feels fatigue as the body has to lungs and accessory muscles work harder to fulfil the oxygen demand. Ineffective breathing pattern may develop due to altered rate, timing, depth and rhythm of breathing (Mete et al., 2018). The respiratory system is unable to supply adequate amount of oxygen to the body and the patient experiences variations in thoracic and abdominal pattern and respiratory rate.
These pathogenic mechanisms lead to airflow obstruction, pulmonary hypertension, hypersecretion of mucus, abnormalities in gas exchange, ciliary dysfunction and other systemic effects (Brashier & Kodgule, 2014).
The clinical manifestations of Mr, Krum’s health include Shortness of Breath (Dyspnea), chronic productive cough with sputum, wheezing, tachypnea (rapid breathing) of RR 30, reduced oxygen saturation 87%, and use of accessory muscles while breathing. The diaphragm and intercostals muscles fail to provide sufficient ventilation to the lungs, therefore the body has to use accessory muscles to improve breathing. His BP and HR is increased. He also shows the symptoms of mild fever.
