This is an individual assessment. Students must not collude with each other. Option 1: Provide a written reflection based on your professional experience to analyse a breach of patient safety in a healthcare setting and explain how quality and/or safety improvements can lead to better health outcomes, with reference to the identified breach. Your reflection should include an evaluation of how the breach was or should have been addressed from an ethical perspective. 9 Version: Semester 1, 2019 OR Option 2: Explain the evolution of National Safety and Quality Health Service Standards (NSQHSS) in the Australian health system and provide a written reflection on how NSQHSS are positioned to improve health outcomes within an overall healthcare governance system. It is expected that students will draw upon unit content and source scholarly literature to critique and evaluate theoretical principles and concepts of quality and safety in the healthcare setting. This written reflection is to adhere to the academic conventions of writing. Presentation: • Each submission should be saved with the file name “HLSC604 Assessment; Surname; First Initial; and student ID [e.g., HLSC604 Assessment 3 Smith R; ID number]. • Provide student name and ID in the header along with page numbers in the upper right hand corner on every page of your assignment, not including title page. • Each submission should include a title page including the unit code; your name and student number, title of assignment, date submitted and word count.
