I have read through all available resources but I am finding the instructions for the action plan section unclear.


I am unsure of where to place the smart goal and the table related information (objectives, strategies, activities, resources, time, measurement).



Do we place the smart goal in the 100 words box titled "action plan", and the table related information in the or the 650 words box titled "How will you achieve this in your next placement"?


Or is the"How will you achieve this in your next placement" box for an essay style answer of that question? while the

In my 5 week NCR placement in ICU, I aim to be confident in effectively communicating with multi-disciplinary teams, such as doctors about my patient's conditions and care. By the end of my placement on the 03/11/2019 I will be able to effectively communicate with multi-disciplinary teams.

Objective:


To confidently communicate effectively to multi-disciplinary teams such as doctors about patient care, to enhance my patient quality care.



Strategies:


Strategies for improvement include:



  • Involving myself when my patient has multi disciplinary teams around, such as doctors, social workers, speech pathologists. Health professionals should function as a team to deliver comprehensive care that addresses as many as the patients needs as possible (NSW Government). This will enable me to engage in conversations about my patients and involve myself to act within the patients health care team.




  • Communicating with the multi disciplinary professionals after they have seen my patient. By communicating with other health professionals involved in my patients care, after they have seen the patient, I will be able to improve my care towards the patient. I will know what is going on, what is planned, if anything has changed in the patients care plan. I will also be able to clarify anything that the doctor or health professional has said, if my patient does not understand. Epstein (2014), explains that enhanced communication between different levels of health care professionals, can reduce mortality and also improve patient quality care, along with health care professional satisfaction.




  • Getting to know my patients, their medical history and current condition, so I know what to discuss with multi disciplinary teams. By reading through my patients notes, I will be able to find out what type of care my patient requires. I will then be able to communicate any concerns I have with the patients doctor, this will in turn help enhance the patient care in which I provide.​​​​​​​




  • Ensure I know who my patients doctor is along with any other multi disciplinary team they are involved with. Knowing who the doctor is, is extremely important so I know who to call or page if my patient suddenly deteriorates or if there is an emergency.



Activities:



  • I will need to attend and participate in patient handovers, to gain an understanding on my patients conditions, in which I can communicate my understandings or concernsto the multi disciplinary team.

  • Page the patients doctor when needed, for example if my patient requiresa Standard Clinical Review due to one of their vital signs being out of parameter I would page their doctor and communicate the issue. Ghiyasvandian, Zakerimoghadam and Peyravi (2015),states by professionally communicating in a clinical setting, health professionals build relationships with each other and become more confident in exchanging knowledge and making clinical decisions about patient care.

  • Include myself when the Doctor or other health professionals are with the patient. When the patients doctor is doing the rounds to their patients, I will aim to include myself in the room if allowed.

  • Read through patient notes and history. This will allow me to gain an understanding on my patients condition so I am able to voice any concerns I have to the multi disciplinary team.

  • Patientfiles including current notes and medical history

  • The computer to access the hospital paging resources

  • Patient notes to find out their doctors name.



Timeline:



  • In my 5 week NCR placement from the 30th of September to the 3rd of November.



Measure of Success:



  • My interim and final assessments on PebblePad completed by my preceptor will state that I can effectively communicate with multi disciplinary teams. I will also achieve a score on the ANSAT toolof at least a 4 for this standard

  • I will feel confident in communicating with multi disciplinary teams such as doctors, about my patients care.

My question was what to place in the 3 boxes.

If we place the action plan with information:objectives, strategies, activities, resources, time, measurementin theaction plan 100 words max box,what do we place in the"Identify how will you achieve this in your next placement"box?
I asked how you would like us to answer the"Identify how will you achieve this in your next placement"box and whether you'd like an essay style answer of that question?
If so, the action plan box is 100 words and I think we would take more than that to cover theobjectives, strategies, activities, resources, time & measurement.Thats why it appears as though we place out smart goal of 1-2 sentences in the action plan box and theobjectives, strategies, activities, resources, time & measurementanswer in the"Identify how will you achieve this in your next placement"box as its 650 words.
Apologies for not understanding, I just need more clarification.

I would steer away from using a table but simply write under headings

I have a question regarding the mapping template.


I understand we are required to write a summary that includes all clinical cues. However, are we supposed to tick the score we got for each cue, or the standard as a whole? I thought it was each, however, the sample template does not do this.

TEACHE


You can just average the scores you received for the individual aspects of the standard and discuss your overall score for the standard or you can select aspects of the standard to focus on.


This might be the areas where you performed less well

We are instructed to do a deep reflection for each standard but I am finding it challenging to do a deep reflection in such little word count. Part One
    The learning outcomes of nursing practicum were applicable in one of the critical situations faced in dealing with the adverse drug reactions. The patient was carried to hospital after acute pain in the left side of chest. Clinical observation showed that the patient had initial symptoms of cardiac arrest and he required immediate assistance. At first, he was received
medication to keep his blood pressure in control. The patient had a history or having hypertension and diabetes type two. He was on medication for the same for the last fifteen years. Also, he was allergic to the blood thinning medications. However, due to the gap in communication or by any other unforeseen condition in keeping his medical records, the condition of ‘being allergic’ to the blood thinning medication went misplaced.
    The patient successfully recovered from the surgical procedure; balloon therapy was given to avoid further complications related to cardiac arrest. When the patient was shifted in the ward, his daily record did not mention anything about that blood thinning allergic issue therefore; two drugs were prescribed by the expert, twice a day to keep his blood running well. The proper dose of the drugs was to include 10 mg a day. Again, due to the technical error, the 10 mg was documented as 20 mg. when the patient was given first dose of thee regular medication, he began feeling suffocated in the ward, started to sweat heavily and there were red bruises all over his body. It was unexpected; however, the patient inquired if he was given any blood thinning medicine and upon being confirmation, he declared that he was allergic to the same.
    From the condition observed of the patient, I learnt that practical understanding of communication constitutes to a large extent in the overall wellbeing of the patients. What makes this part important can be referred to the practical situation in which, the missing of information could have been fatal to the life of patient which was related to the adverse drug reactions. Not only the patient but also the hospital staff was responsible for creating that emergency. Medical history examination is another mandatory concept which the professionals must go through. They should follow the National Safety and Quality Health Standards section 4.7 and 4.8, proper management of the adverse drug reactions through keeping the information and sharing at high priority.
    I also came to discover the importance of mental preparedness in order to deal with the issue of critical condition of the patient; at times, it is implied that unexpected emergency situations emerge out of nowhere and it becomes the responsibility of the nursing professional to make practice assessment of the patient. This assessment is crucial during emergency phase because nursing professional carries out identification of the issue, its proper record and information sharing management with the clinical experts. Therefore; mental stability and professional attitude both are significant to deal with the problems that confront the nursing practice in emergency conditions.
Part B
    Based on the observation of the above situation, the NQHS standard 4 sections 4.7 and 4.8 are applicable. It states that “The clinical workforce documenting the patient’s previously known adverse drug reactions on initial presentation and updating this if an adverse reaction to a medicine occurs during the episode of care” (NQHS, 2020). Under section 4.8 of the standard, it has been written that “The clinical workforce reviewing the patient’s current medication orders against their medication history and prescriber’s medication plan, and reconciling any discrepancies” (NQHS, 2020).
