Assessment 2: Case Study Your task is to: develop an essay that outlines a care plan for a selected case study. demonstrate your ability to understand the cultural needs and personal experience of the person with a diagnosis of a life-limiting condition, and demonstrate an evidence-based approach to assessment and clinical decision making for this person’s care. The case study requires you to: think critically and creatively about the selected health needs of the person, review concepts and principles related to palliative care. For this assignment, you are required to: develop an essay that outlines a care plan which outlines potential nursing interventions for managing these two symptoms (pain and breathlessness) that are acknowledge the cultural needs of the patient provide an evidence based rationale for the interventions you propose. provide a description of how you would monitor and evaluate the effectiveness of the interventions within the context of palliative care. The emphasis is on a clear understanding of the individual’s palliative needs and the context of care. Your work should demonstrate an evidence-based approach through the use of relevant literature, systematic reviews, clinical care guidelines and other materials as appropriate. This will require you to use the CINAHL, Medline and/or Meditext databases, Cochrane library, as a minimum, to retrieve information. Tom is a 55 year old Aboriginal man with advanced lung cancer and multiple metastases. He collapses at home, his family call the ambulance and he is admitted to the ward, extremely breathless. His disease is now end stage. Tom’s wife Cec and their son Jimmy are with him in the ward. His main issues are pain and breathlessness. Your care plan and interventions are to address these main symptoms using a patient centred, individual approach.
