An 18-year-old Hispanic female is brought to the ER by her boyfriend for a fever of 104.5°F and shaking chills. Your questioning reveals that she drinks socially and uses IV drugs. On physical exam, crackles are evident throughout her lungs. The attending physician suspects pneumonia, admits the patient to the hospital, and starts treatment with antibiotics. The next day, during your morning rounds, you note additional physical findings: Your patient now has tiny oral, conjunctival, and subungual hemorrhages. A new-onset cardiac murmur is also apparent. The patient is taken for an ECHO, but the images are inconclusive. Blood cultures are also negative. How should this patient be treated?
