Albert Lewis, a 52-year-old hospitalized patient, is receiving multiple medications. While in the hospital, Albert receives the following drugs: • Nitroglycerin tablet sublingual prn for chest pain • Nifedipine Extended Release (ER) orally every 6 hours • Mycelex Troche twice a day • Clorafan 500 mg intravenously (IV) • Insulin subcutaneous injections every morning • Garamycin optic solution, 2 drops to each eye, 3 times per day • Fentanyl transdermal patch every 12 hours.
1. How would you respond when the patient asks you, “Why can’t I take all my medications orally?” 2. Discuss the advantages and disadvantages of each route for each of the medications listed above. 3. What special precautions should you take with drugs with an enteric coating and those that are extended release?
