A summary of our clinical innovation To design a mobile application to aid with management of diabetes type 1 in adolescents. Functions of app 1. Adolescents will use a machine called accucheck to take a sample of their blood to check their sugar level. This result will be automatically captured in real time in the mobile app. • This is to ensure misreporting of results. • Parents are able to monitor their blood results. • This will be a library of results that can be shared with healthcare professionals. 2. This mobile app also acts a support group. • Adolescents and parents are able to ask questions related to disease process, diet, and exercise on an FAQ portal that will be mended by diabetic nurses. 3. This app also has a function that counts calories and calorie burner. • This function is will be useful when adolescents visit their dietician. The reasons for the clinical innovation • Adolescents will gain autonomy over their disease process and will be empowered with adequate knowledge on disease management. • Better disease management will lead to a decrease in tension in parents and adolescents. • To increase emotional and social support. Assignment Details Students are to develop a clinical innovation implementation plan for the clinical innovation that was researched in the first assessment. Building on from the first assignment, students will provide a Clinical Innovation Plan that reflects the issues and opportunities that emerge from the proposed innovation. The Clinical Innovation Plan will follow a format that is comprehensive and provides the reader with information relating to: • The innovation that is to be implemented (Pls refer to attached file:annotated literature) • Change management considerations that need to be integrated into the Clinical Innovation Plan (To include these steps) Change management process .The change management process is the sequence of steps or activities that a change management team or project leader would follow to apply change management to a project or change. Based on Prosci’s research of the most effective and commonly applied change, most change management processes contain the following three phases: Phase 1 - Preparing for change (Preparation, assessment and strategy development) Phase 2 - Managing change (Detailed planning and change management implementation) Phase 3 - Reinforcing change (Data gathering,corrective action and recognition.
