Specific Requirements of Kathryn
Kathryn, aged 64 years, has been suffering from Type 2 Diabetes Mellitus (DM) since the last 10 years, but has not been following the prescribed DM management regimen properly; according to the expected outcomes of her General Practitioner (GP). Consequently, she has developed chronic diabetic pedal neuropathy. Hence, the most important concern of Kathryn, requiring immediate care post-discharge is her diabetes and foot care. Further, since she was also amputated of her three foot-digits due to developing gangrene from diabetic foot ulcer, therefore, curing the pain, preventing hospital-acquired infection, post-discharge complications and managing her DM medications would also stand important.

    I- Include patient and family as full partners in discharge planning process
    Kathryn and her family will be included in all meetings about her discharge. Amongst them, one will be identified to provide her with post-discharge care at home.
    D- Discuss with them key areas to prevent problems at home
    As mentioned by Noll and Russell (2016), in order to prevent the possible occurrences of post-discharge complications at home, the patient and their family need to be aware about the five key areas— forthcoming life at home, reviewing medicines prescribed to the patient, making them cautious about the probable issues as well as signs, explaining the results of the diagnostic tests and make appointments for the patient for follow-ups.
Hence, for Kathryn too, these steps need to be followed. She and her family will also be informed about the environment that would be the most suitable for her to live in the home, after being discharged, the list of medicines will be reviewed with them in terms of their usefulness, possible side effects, method of administration, dosage and timings. As supported by Caughey et al. (2017), if any possible signs are noted of complications or infections then the patient is to be immediately reported for readmission and a regular regimen of check-ups should also be maintained.
