A client in her first trimester of pregnancy asks the nurse which medications should be avoided during pregnancy. The nurse s response is based on the knowledge that during pregnancy:
1. Most over-the-counter medications are safe.
2. When possible, drug therapy is postponed until after pregnancy and lactation.
3. It is wise to discontinue all drugs used in treating medical conditions.
4. The decision whether or not to take medication is the responsibility of the woman.
