A 68-year-old man has had a repair of a fracture of his right tibia. During the evening of his second postoperative day, he complains of increasing pain and slight paresthesias of his right toes. Opioid analgesics have only moderately relieved his pain. The night-shift nurse documents her findings and asks you, as the oncoming day-shift nurse, to report these findings to the orthopedic surgeon when he makes his rounds. You assess the patient after report and find that he now describes his right lower leg as “feeling tight” and that he has sensations of “pins and needles.” His right lower leg has a capillary refill longer than 3 seconds, and his right toes feel much cooler than the left toes. What additional assessments might you make at this time? What is your priority nursing diagnosis and intervention?
