Background/Overview During your work placement, you will be undertaking a variety of duties and tasks that relate to your course and the role of a counsellor or case worker. You will need to successfully demonstrate that you can perform tasks required in the work environment and collect evidence. This portfolio will be broken down into tasks, and they all need to be completed in accordance with the Work Placement Plan agreed upon with your Workplace Assessor. Your Workplace Supervisor will give you guidance and feedback on your performance, supporting you to complete the tasks in an appropriate and professional manner. Evidence of your performance must be collected and submitted to your Workplace Assessor. This document, the Workplace Assessment Portfolio, is where Tasks 1-6 are outlined with instructions on how to collect the required evidence. 2. Portfolio Information You will be required to complete each task in this portfolio at your Host Organisation under the supervision of an approved Workplace Supervisor (See Workplace Assessment Guide). Role of your Workplace Supervisor Your Workplace Supervisor will sign off on the demonstration of these tasks to confirm that you have completed the tasks in accordance with the Host Organisations policy and procedures and operational standards. Your Workplace Supervisor’s role is not to judge your competence for the relevant unit/s of competency. Role of your Open Colleges Workplace Assessor Your Open Colleges Workplace Assessor will talk to your Workplace Supervisor about all the tasks. Your Supervisor will confirm that you have completed each task according to the task instructions so you need to ensure that they witness each task as you complete them. De-identifying workplace documents Any workplace documents that you use for your portfolio task submissions must be de-identified. Make sure you have completed all the tasks and upload your work to OpenSpace. 33472/PA CHC51015 Diploma of Counselling (E1202) Page 3 of 28 Task 1: Complete Client Case Notes and Develop Mental Health Treatment Plans Instructions: Documenting and reporting according to organisational policies and procedures are a significant process of the counselling practice. There are two parts in this assessment task. Part A: Complete client case Notes Select three (3) different clients you have worked with at your work placement and complete three (3) separate case notes for each client documenting your session with the client according to your host organisation guidelines using a case note template available from your organisation. If such template is not available, you may use the one provided in Appendix 1. In each of your case note, ensure all the following is addressed: • Presenting issue/problem • Assessment • Intervention • Plan Part B: Develop mental health treatment plans Develop three (3) mental health treatment plans for three (3) different clients according to your host organisation policies and procedures using a treatment plan template available from your organisation. If such template is not available, you may use the one provided in Appendix 2. In your plan, ensure you cover the following: • Client information • Presenting issue(s) • Client history • Medications • Any other relevant information • Result of mental state examination • Risks and co-morbidities • Treatment plan to include: issues, goals, interventions and referrals • Strategies for crisis intervention 33472/PA CHC51015 Diploma of Counselling (E1202) Page 4 of 28 Task 2: Legal and Ethical Considerations Instructions Legal and ethical issues can vary depending on the industry you work in. However, having a good understanding of the impact of legal and ethical issues on your counselling practice will guide you through your decision-making process. There are two parts to this task: Part A: Develop an induction presentation for legal and ethical requirements In consultation with your workplace supervisor, develop a presentation for clients as part of their induction/orientation to the organisation and services provided. The presentation should last approximately 15- 20 minutes. You can design your presentation in any way you would like. For example, it can be a PowerPoint presentation (maximum of 10 slides), a presentation with use of pictures, a flow chart, or infographic, etc. Address three of the following legal and ethical requirements: • Codes of conduct • Contract requirements • Discrimination • Duty of case • Practitioner/client boundaries • Human rights • Mandatory reporting • Informed consent • Privacy, confidentiality and disclosure • Records management Ensure your procedures display industry currency and that your supervisor would approve the presentation to be used for service users. Part B: Presentation and reflective journal Present your induction/orientation presentation you have developed to your workplace supervisor or team leader and ask for their feedback on your presentation and communication skills. Write a Reflective Journal entry (approximately 300-500 words) on the feedback they provide. Your reflection must include: • What feedback was provided? • How did you respond to the feedback? • Address 3 different legal and ethical issues that are relevant to your work role and discuss 2 potential work practice improvements to enhance workplace responsiveness to legal and ethical requirements. • Based on the feedback, what changes will you make to the presentation? • Based on the feedback and the needs of target audience, what changes will you make to your communication techniques? 33472/PA CHC51015 Diploma of Counselling (E1202) Page 5 of 28 Task 2 - Part B Reflect on and evaluate the induction/orientation presentation and communication skills (approximately 300-500 words) Reflect on the induction/orientation presentation you developed and the communication skills you used during the presentation. Your reflection must include: • What feedback was provided? • How did you respond to the feedback? • Address 3 different legal and ethical issues that are relevant to your work role and discuss 2 potential work practice improvements to enhance workplace responsiveness to legal and ethical requirements. • Based on the feedback, what changes will you make to the presentation? • Based on the feedback and the needs of target audience, what changes will you make to your communication techniques? Student reflection: 33472/PA CHC51015 Diploma of Counselling (E1202) Page 6 of 28 Task 3: Recognise and Respect Social and Cultural Diversity Instructions Understanding social and cultural differences between yourself and the people you work with is an important part of your counselling practice. Reflection of your own social and cultural perspectives is equally important. There are two parts to this task. Part A: Interview 3 different people In consultation with your workplace supervisor, select and interview 3 different people (they could be either clients or work colleagues who come from different social or cultural background than yourself) in your workplace. Take notes during the interview of what each person says. Select a relevant topic to discuss or use suggestions from the following list: • Differences in social perspectives • Social, political and economic issues • Cultural values • Religious beliefs • Attitudes towards disability • Attitudes towards gender, including transgender • Benefits of diversity in the workplace Before you conduct these interviews, create 3 questions that you will ask each colleague/client and write down their responses during the interview. Ensure you advise your colleague/client of an option for topic that would be culturally appropriate to them and in line with their goals and needs. The interviews should be maximum of 3 minutes long each and it must demonstrate that you are able to recognise and respect diversity.
