Nurses also have a professional responsibility to maintain up-to-date practice.1 This paper provides a guide on how to critically appraise a qualitative research paper. What is qualitative research? Qualitative research concentrates on understanding phenomena and may focus on meanings, perceptions, concepts, thoughts, experiences or feelings.2 Qualitative research examines how or why a phenomenon occurs. It collects data in the form of words, texts and or images via interviews, observations, photographs or document reviews. Qualitative research does not use discrete vari- ables like those used in quantitative approaches. In crit- ically appraising qualitative research, steps need to be taken to ensure its rigour, credibility and trustworthiness (table 1). Some of the qualitative approaches used in nursing research include grounded theory, phenomenology, ethnography, case study (can lend itself to mixed methods) and narrative analysis. The data collection methods used in qualitative research include in depth interviews, focus groups, observations and stories in the form of diaries or other documents.3 Authenticity Title, keywords, authors and abstract In a previous paper, we discussed how the title, keywords, authors’ positions and affiliations and abstract can influence the authenticity and readability of quanti- tative research papers,4 the same applies to qualitative research. However, other areas such as the purpose of the study and the research question, theoretical and concep- tual frameworks, sampling and methodology also need consideration when appraising a qualitative paper. Problem Purpose and question The topic under investigation in the study should be guided by a clear research question or a statement of the problem or purpose. An example of a statement can be seen in table 2. Unlike most quantitative studies, qual- itative research does not seek to test a hypothesis. The research statement should be specific to the problem and should be reflected in the design. This will inform the reader of what will be studied and justify the purpose of the study.5 Literature An appropriate literature review should have been conducted and summarised in the paper. It should be linked to the subject, using peer-reviewed primary research which is up to date. We suggest papers with a age limit of 5–8 years excluding original work. The liter- ature review should give the reader a balanced view on what has been written on the subject. It is worth noting that for some qualitative approaches some literature reviews are conducted after the data collection to mini- mise bias, for example, in grounded theory studies. In phenomenological studies, the review sometimes occurs after the data analysis. If this is the case, the author(s) should make this clear. Theoretical and conceptual frameworks Most authors use the terms theoretical and concep- tual frameworks interchangeably. Usually, a theoret- ical framework is used when research is underpinned by one theory that aims to help predict, explain and understand the topic investigated. A theoretical frame- work is the blueprint that can hold or scaffold a study’s theory. Conceptual frameworks are based on concepts from various theories and findings which help to guide the research.6 It is the researcher’s understanding of how different variables are connected in the study, for example, the literature review and research question. Theoretical and conceptual frameworks connect the researcher to existing knowledge and these are used in a study to help to explain and understand what is being investigated. A framework is the design or map for a study. When you are appraising a qualitative paper, you should be able to see how the framework helped with (1) providing a rationale and (2) the development of research questions or statements.7 You should be able to identify how the framework, research question, purpose and literature review all complement each other. Sampling There remains an ongoing debate in relation to what an appropriate sample size should be for a qualitative study. We hold the view that qualitative research does not seek to power and a sample size can be as small as one (eg, a single case study) or any number above one (a grounded theory study) providing that it is appro- priate and answers the research problem. Shorten and Moorley8 explain that three main types of sampling exist in qualitative research: (1) convenience (2) judgement or (3) theoretical. In the paper , the sample size should be stated and a rationale for how it was decided should be clear. Methodology Qualitative research encompasses a variety of methods and designs. Based on the chosen method or design, the findings may be reported in a variety of different formats. Table 3 provides the main qualitative approaches used in nursing with a short description. Procedure The authors should make it clear why they are using a qualitative methodology and the chosen theoretical approach or framework. The paper should provide details on O ctober 6, 2020 by guest. P rotected by copyright. http://ebn.bm j.com / E vid B ased N urs: first published as 10.1136/ebnurs-2018-103044 on 1 D ecem ber 2018. Methods of data collection should be identified and be appropriate for the research statement/question. Data collection Overall there should be a clear trail of data collection. The paper should explain when and how the study was advertised, participants were recruited and consented. it should also state when and where the data collection took place. Data collection methods include inter- views, this can be structured or unstructured and in depth one to one or group.9 Group interviews are often referred to as focus group interviews these are often voice recorded and transcribed verbatim. It should be clear if these were conducted face to face, telephone or any other type of media used. Table  3 includes some data collection methods. Other collection methods not included in table  3 examples are observation, diaries, video recording, photographs, documents or objects (artefacts). The schedule of questions for interview or the protocol for non-interview data collection should be provided, available or discussed in the paper. Some authors may use the term ‘recruitment ended once data saturation was reached’. This simply mean that the researchers were not gaining any new information at subsequent interviews, so they stopped data collection. The data collection section should include details of the ethical approval gained to carry out the study. For example, the strategies used to gain participants’ consent to take part in the study. The authors should make clear if any ethical issues arose and how these were resolved or managed. Table 1 Useful terms Term Description Credibility Credibility is establishing the study’s findings with reality. This demonstrates the believability or truth of the findings. It ascertains whether the study findings are believable based on the participants’ original data and participants’ views. Confirmability Confirmability questions how the study findings are supported by the data. It identifies any bias that may have been present. It is the level to which the findings can be confirmed or corroborated. Confirmability is concerned with determining that data and interpretations of the findings are not made up by the researcher’s imagination, but clearly derived from the data. Member checking This is a technique in which the data, interpretations, analytical categories and conclusions are fed back to the participants from whom the data were collected. This process strengthens the data as it allows participants to clarify and correct any errors and provide additional information if necessary. It can also highlight how participants and researchers view the data differently. Rigour This is the systematic approach and techniques used to ensure reliability and validity of the study. Reflexivity Reflexivity is the practice of critical self-reflection about oneself as a researcher. It involves looking at one’s biases, preferences and preconceptions. The researcher should reflect on their relationship to the participant, and how the relationship affects participant’s answers to questions or decisions in the research. For example, how does being a nurse impact on interviewing a patient in your care? Triangulation Triangulation refers to the use of different ways of collecting or analysing data and the cross-checking of these to see if findings from different methods, different analysts, different theories or different reliability checks of sources show similar results. Trustworthiness Trustworthiness is concerned with how the researchers establish the study findings as credible, transferable and confirmable. Transferability This refers to the level of which the study findings can be transferred to other situations. The reader should look at the detail of the study including the methods and rigour applied and compare this to their own situation to determine if the findings can be applied to their own situation. Introduction
This essay is about acute stroke care services, which are researched in Greater Manchester (GM) and London, in 2010. This is criticised for minimising hyperactive acute stroke units. This article is helping to identify the negative impact of stroke care pathways based on the patient's experience. There is supporting e
vidence related to the service of the hospitals.
For improving the access to inpatient stroke care in several countries, there is the existence of centralised acute stroke care services. Even there is the implementation of several strategies to get a better result. The design helps to provide services within 3 days. This article has discussed the importance of patent experience in the hospital (Iyawa, Herselman and Botha, 2016). Then the literature review has also summarised, which include patient's transfer to the hospital and personal care to the patients in the hospital.
This article is important in the field of nursing because the experience of patients mostly depends on the nurse and the way; they are taking care of the patients. Here satisfaction of a patient depends on the quality of care they get from the nurses in the hospital. National Institute for Health and Care Excellence has proven some guidance based on experience, which helps to improve the quality of services in the hospitals (Balk and Lichtenstein, 2017).
Here the impotence of this research is to improve the condition of the hospital that the patients speak in support of the nursing system. Every patient should treat in the same way that the review of the hospital can get better. The evidence of the research is the proof that data collection could be more efficient and there are some points missing in the literature review. Based on nursing practice, the system could be enhanced for better improvement.
Critique of the Article by Perry et al. (2018)
Authenticity
The title of this article is Patient experience of centralised acute stroke care pathways. Participants were happy with emergency services at the hospital. Some Difficulties were faced by families who come to visit hospitals. The researcher found that local hospitals were not always timely. On the other hand, there were no detrimental effects reported. The article it has been found that title, keywords, affiliations and abstract is written in a normal style, but it has several categories, which identify the object (Ramsay et al. 2018).
This article helps to influence authenticity. Apart from that, the author nicely explains the duty and responsibility of a nurse in the hospital. Apart from that, the auteur explained some professional ways to maintain their own responsibility for the betterment of patients and mention practices regarding keeping data maintained up to date.
The research paper helps to improve guidance regarding the improvement of performance that discloses the credibility of the author in this article. The sample and the data collection was done from the urban area, rural area and as well as from the community in the hospital. This gives a clear explanation of the implication. The research paper helps to clear several doubts regarding the field of nursing.
Robustness
Research Problem
The research is conducted based on several problems. This includes the health care experience of the patents. Here several patents have several opinions regarding the service of the hospital. Some people give a review of good services and on the other hand, some people are not satisfied with the service of the hospitals (Boustani, Alder and Solid, 2018). Hence, this research is conducted to find the problems that the stroke patients face regarding the service of the hospital. The research problem helps to identify several other problems during the time of discharge from the hospitals.
Another problem is an insufficient bed in the hospital for what the accidental case have to wait for a long time (Imison et al., 2014). Regarding the transfer of patients' in a different hospital for a better health condition, the stuff of the hospital mostly stays confused with the information regarding the availability of a bed in the local hospitals. The patents often have to suffer regarding the ambulance service to the hospitals.
Literature Review
The literature review includes peer-review and this helps in the primary research. The literature review helps to summarise the paper in order to find Patient experience of centralised acute stroke care pathways. Here the literature helps to provide evidence regarding the stages of acute stroke care. Apart from that, this also helps in crosscutting issues, which help the stages of care.
According to the findings of this article by Perry et al. (2018), it has been found that there is a need for an ambulance to transfer the patients to the hospitals. In the hospitals, there are mostly stroke patients and at the beginning, they get emergency services. There is a need for an ambulance to transfer the patients in the hospitals and it is identified that they arrive on time.
On the other hand, it is identified that the ambulance provides great service on the patients of stroke but not the casual cases (Cristea, Cahan and Ioannidis, 2019). It is not discussed in the article how the discharge patents gave suggestions about the improvement of the system. In order to talk about hospital care, the patients generally have good experiences and here they are treated in the general ward. Here according to the article, a person who comes with an emergency accident case is not treated well because of not having enough available bed in the hospital.
The researcher has struggled in the papers with an age limit of 5–8 years, excluding original work. Through the literature review of this article, the author tried to execute the significance care service for stroke patients. In the context of the healthcare system, this literature enhances a positive and strong side (Department of Health, 2007). Here, the delay in the services caused tension and anxiety among the patient. It is seen that recovery takes in the process of treatment.
Here the patients have to stay for a longer span of time in the hospital, which causes frustration and depression. Because of depression, the patients often report for poor services and poor experience in the hospital (Rutberg and Bouikidis, 2018). Most of the stroke patients gave a good review on how they are treated with dignity and respect, but on the other hand, there are other patients, who complain regarding the...
